Why this order
The four stages are not a style preference. Each one exists to prevent a failure that occurs reliably when it is skipped or taken out of sequence.

| Learn | A question that data can answer, and a definition somebody else could use. | Data are collected that nobody can use, against a definition that quietly changes. |
| Observe | A record with no conclusions in it. | Memory reconstruction, which drifts towards whatever you expected. |
| Understand | A finding with its limits named. | An opinion presented as a finding — and inherited as one by everybody downstream. |
| Apply | A change with an owner, a measure and a review date. | Five simultaneous changes, an improvement nobody can attribute, and nothing reproducible. |
The rule that holds it together
No stage may borrow the output of a later stage. You may not act before you have read, and you may not read before you have observed.
